• Activity/Sports registration cannot be completed without a valid physical on file.
• Physicals are good for three years from the date of exam.
  • A copy of a visit summary does not qualify as a sports physical; a completed MSHSL sport physical form (or clinic specific letter) signed by your provider is required.
• Free/Reduced Benefits - Athletes should apply for benefits prior to registering.
  • Visit the Free/Reduced Application page to apply.
  • Contract Tyra Schlichting from the Nutrition Services Office with questions.
• All non-athletic activity and sport fees (with the exceptions of summer strength training & clay target range fees) count toward the annual family cap of $625.
