A visit summary does not qualify as a sports physical. A completed MSHSL sport physical form (or clinic specific letter) signed by your provider is required.
- Free/Reduced - Athletes should apply prior to registering.

- Contract Sue Fibranz from the Nutrition Services Office with questions.
- Sports and non-athletic activities fees (with the exceptions of summer strength training & clay target range fees) count toward the annual family cap of $625.
