From the Health Office: Medications Dear Parent/Guardian(s):The end of the school year is approaching! If your child has medication stored in the health office (including prescription/non-prescription medication, EpiPen, diabetic supplies, etc.), a Medication Release Consent Form will be sent home Friday, May 16th. Please check your child’s folder to ensure you receive it. In order for the nurse to send your child’s medications home on the last day of school, the form must be completed by Tuesday, May 27th. If the form is not completed by this date, a parent or guardian will be expected to pick up the medication by 2:45 p.m. on the last day of school, May 30th. After that time, all remaining medications and supplies will be destroyed. Please note that controlled medications (such as Ritalin, methylphenidate, Valtoco, Adderall, etc.) WILL NOT be sent home with students and must be picked up by a parent or guardian by 2:45 p.m. on May 30th. If you have any questions, please feel free to reach out.
Thank you,
